New patient registration

If you live within our practice area, you are welcome to register with us and our reception staff will be happy to guide you through the process. All you need to register is your photo ID (passport or driving licence) and proof of address (such as a bank statement or utility bill).

To register, you need to complete a registration form (GMS1) and a health questionnaire, which can be collected from reception at either Practice. The questionnaire provides useful information to our GPs while we await your full registration.

When you register, you will be offered a health check with a member of our healthcare team. This provides us with up-to-date information about your health status and allows us to take accurate note of any repeat prescriptions you may require.

Medical treatment is available from the date of registration. Please contact our reception for further information about this.

Named GP

All registered patients are assigned one named GP, who is accountable for their care. For more information or to find your named GP, please contact reception.
